Dio was still healing from his body transfer, and his stand wasn't able to stop time for more than a few relative seconds, with intervals in between because he couldn't sustain the energy. Given that Dio was also up against some of the fastest and most precise stands in the series at the time, even his time stop would only let him get so far if they knew where he was. One wrong move and Dio knew he would've been downed. He respected the strength and will of the Joestar line, if he 'did' try to just jump in and instakill them, he might have gotten one or two of them, but he'd be within range of several other incredibly dangerous stands. So he had to play mind games to pick them off one by one, so they wouldn't catch on to his technique. And of course, Kakyoin is absolutely awesome so they still managed to learn it.

If the flash didn't know what was going on then Maid in Heaven would eventually just reach capacity to restart the universe as a whole, though if the Flash is so overpowered he can live the instance of the universe's life span in a single second (essentially) then I guess he'd just be able to see it coming even so much as a second before it actually happened and stop it. I mean, Maid in heaven accelerates infinitely but the flash is just always moving at infinite speed I think.
