Each jersey will be $74.99. Shipping is free within the US, +$20 everywhere else.
Here's how it's going to work:
1. Sign up here: http://derpy.me/pv3lb
2. Signups close Saturday the 12th.
3. Shortly thereafter I'll email you an invoice where you can pay online with a credit card
4. We'll pool the money, pay the manufacture, and place the order
5. Six weeks later the jerseys will be finished and ready to ship out to you

Definitely sign up if you want some jerseys, this isn't exclusive to a couple people. But because of how I'm running this one I can't push any of the deadlines back. Signups *will* close the 12th. When payments are due (next week sometime), you *will* have to pay, or your order will be dropped.
Basically if you don't have the money right now, don't sign up. People who signup but don't pay mess up the numbers for everyone else.

I haven't touched CoD in like 4 years, so I haven't played black ops 3. Even then, I'm not that highly skilled of a modder. Texture and sound replacements are as far as I really ever go.
A fully custom modeled map is beyond my skill level unfortunately. There are many others that are good in that area though and I'm sure someone will end up doing something like that.

As Rag said, Tony deals with aesthetics (Don't expect him to help you though, he has a reputation of being a considerably useful crazy killjoy). However, there also are tutorials on the wiki and if you read through all the pages you'll notice that PES 16 is not set up fully at all for now. Stuff works, but it's rough. And /nosaveeditor/ is probably going to kill '16 for Winter.
