Find mechanics you like, tweak them as you like, and get some criticism on them. Mechanics are weird.
Premade, but give the players some choices like to the race, sex, GENERAL appearance(so if you want this character to have piercings, do it but don't let the players spend one thread deciding whether their taint or ballsack gets pierced), general abilities, etc.

Make it simple. Personally, the only thing deciding outcomes in my quest is a basic stat system and a d20.
More than half the time, I don't even use it.
Pre-made is usually better, but there's nothing wrong with letting the thread decide.
Learn from watching and analyzing successful quests.
Other than that, just try, learn as you go.
If it bombs, you can always try again with a different trip.

Alright I've got some legitimate questions today.
In my current CYOA I've been talking a lot more time at the front end of the quest to set up the main character and setting before having the world actively try to kill them. Something I've noticed is that the players have been completely ignoring the prompt to instead only ask questions about the status of the MC's life or the one side character that doesn't apply yet in the story. What I've been doing in those cases is have the MC stall for a bit and give a little internal monologue on the subject before representing the original prompt.
Should I be worried about this? Does this mean I'm doing things wrong or handling things wrong?

If your players are interested in the main character enough to want a tangent, let them continue along that line. Answer their questions, flush out your characters.
