Not bad, but that 'twist' seemed kind of pointless. Didn't really add anything to the story and in fact took a lot away from my interpretation. Mental illness doesn't have a 'reason' or direct cause in 90 percent of cases.
fucking awesome book. I wish I could find something I enjoy as much as it besides story of the eye.

I hated the pop psychology interpretation of Frankie's actions tacked onto the ending.
Also unimpressed with why the brother went nutzoid. You work at a hospital, then toughen up you fag.
I liked all the weird ritualized slaughter of animals and insects though.
