Not a novel but the Death of Ivan Illych by Leo Tolstoy was pretty gruesome. I've heard that doctors who read the story say it's pretty much to the tee of how Ivan's disease should be described.

This, 100%. One of the absolute, most soul-crushing books I've ever read.It has a happy ending however (fortunately).
