A lot of transgender people just want to pass. They don't want to make it the center of their existence, they just want to assimilate into normal gender roles, just not the one that matches their bits. While I personally am not transgender and quite fine with breaking the gender mold in many ways I realize it's not that easy for other people in other circumstances. Society can be actively cruel towards people not fitting the gender norm. To answer your point, no it is not a fantasy. Of course not everyone will judge me by the things I want them to, but it doesn't matter.
If an employer sees that I will be an asset to them, then they don't care at all about my looks. Maybe a small number will, but I'm not desperate and there will be lots of others who won't care and will hire me.
In a personal relationship I would not want to be with someone who places such a high value on looks. If someone does not want to be with me solely based on the way I look, then that is definitely not someone I want to be with.
I have lots of friends and I know none of them care about my looks because we are friends based on similar interests. A friendship or a relationship can't be based solely on appearances anyway, so I would never want find myself in a situation were I am in a relationship with someone who is only with me because of my looks. It would end badly.
