Slow reader reporting in, often become distracted with other thoughts while I am reading or after half an hour find myself slowing down.

read interesting things exclusively. do that for a really long time and read at least two or three hours a day. doesn't matter what, as long as it's a book and you're very interested in it.

It occurs only when reading or particular activities? If the answer is yes, you're fine unless it is a problem for your life, in which case you should consider getting into behavioral therapy. If this has been happening since you remember, consider the possibility of adhd. There is medication, but it isn't meant to be taken unless necessary - most people with adhd simply live with it.
