The problem is that 99% of the MtF's you see on 4chan are passable but in reality most are not. You have a distorted image of what a real MtF looks like.
I do health services in the LGBT community and can tell you that the vast majority of MtF's I see are not - barely passable. In my work I've maybe seen 2-3 that I felt were cis female before talking to them.
Now that's not saying MtF's are bad. The majority of them were comfortable with where they were in transition.

I'm going to guess 35-40. Our clinic sees about 150 clients per week, primarily LGBT, in a fairly large city.
Not scientific but I'd say 5% were high-level feminine in looks and personality. 40% did not pass or were borderline but even those were feminine personality wise. Remember a personality can change faster than a body and most of these feminine personalities are what drove the physical change.
The other 55% are somewhere along the spectrum of a physical femininity from not quite passable to fairly close. Again, the majority are feminine personality-wise.
