Simply put, a good transition requires three ingredients:
-Young age
-Good starting point
-Money
Of those who are young (let's say, below 30), not many MtFs have a "great" starting point. I mean, just consider the ribcage size alone. How many MtFs have an underbust around 30"? Probably not many. 36"+ seems to be more common. Then there's the ones that are freakishly tall (6'3"+).
If you fulfill the first two criteria, and therefor have a passable body, you also need a passable face, and not many people who have gone through male puberty have an androgynous face. You need a lot of money to fix that with FFS, we're talking tens of thousands of dollars.
So, the amount of people who fulfill all these criteria is very small, I think. It's a combination of youth, luck, and access to money.

My experience as a cashier in Seattle, I spotted just about as many men who had FtM tells, as I did spot MtFs. Maybe like, one total more MtF who clearly didn't pass, out of a pool of about 4, over a period of some months.
