Most transsexuals make the effort to effectively become intersex in order to adopt as many elements of the other sex's biology as they can. Here's my line: a brain raised on the chemical diet of one sex then masculinized/feminized via hormones becomes distinctly different from the brains of both men and women and has its own set of behavioral tendencies. If we make effort to enumerate the different gender behavior tendencies of individuals based on their sex - say, for a study - then those brains would deserve additional entries. What is gender but such a profile combined with some culture-specific aspects? I actually think tumblrites would throw a fit at such a biological view of gender.

It depends on how much hormones are in your system then. If you have a low amount then you can be more neutral in your thinking but they have looked at cis brains and trans brains and there are clear cut white matter differences between males and females even with the hormones at play.

There are two distinct genders that signify the typical behaviours of the biological sexes. You don't have to invent all these additional attention-seeking genders because you behave slightly atypically of your biological gender.
In the case that a individual of a specific gender experiences gender dysphoria strong enough that it interferes with their day to day life, then they can either (if they're younger than x age) get hormone treatment and gender reassignment in order to change their outward appearance to reflect the other gender, or (if they're older than x age) go into therapy and learn to manage the dysphoria.

Ignoring the troll, I'd argue there are four genders: male, female, hermaphrodite, and androgynous. I believe this is sufficient to cover all possibilities without going into retard/imaginary territory.
male/female are obvious, having characteristics of a male or female respectively.
herm would be having characteristics of both, like a pre-op transsexual.
androgynous would be having few characteristics of either: no boobs, light muscle, somewhat feminine face.
Hermaphrodite might be a little redundant. Androgynous would also account for people who have been neutered.
