It's hard to tell, it's not properly documented in any way. I can't imagine third gender being too different from all the people we have here who want to transition but don't really like the idea of being stealth and expecting everyone to treat them as cis.
But it's a social issue, I think. It's weird that it takes so much effort to accept that someone would want to be the opposite gender, with or without the means to actually pass. In societies with a less structured culture I'm sure they just accepted it and moved on, because nothing is lost or gained. I'm not blaiming Europe, India has it's own ridiculous segregation for example.
