just say whatever they look most like and if they bitch it's their problem that they don't pass, not yours. Don't feel guilt because someone looks ambiguous

do I really need to ask their gender every time I meet someone? cis people will think I'm creepy and if I use the wrong pronoun to a trans they might get mad.

Or just refer to them as the gender they look like and if they get offended laugh at them and hand them some pills so they finally act on their suicidal thoughts

Only ask in cases of androgyny.
If they're evidently one gender or the other, stick with that one.

>do I really need to ask their gender every time I meet someone?
Best bet is to observe a little and see what other people use.
People can get seriously offended when you use the wrong pronoun, even cishets.
That and it's awkward when you get it wrong.
>Would it look ridiculous in a skirt?
N - she, her, hers
Y - he, him, his
If it takes offense to that, tell it to fuck off.

Go by what they look like (or specifically what they appear to be trying to present as - if they're a biological male that's obviously trying to look female, even if totally unpassable, it's reasonable to assume they're MTF). Of course, if they explicitly tell you their gender identity or what their pronouns are, use that, but it's better to make a reasonable guess and be corrected than to constantly be asking people what gender they are.
