For long term transition you have to get an orchi or SRS so that you can quit taking an AA.
As you get older if you're still on an AA or have had an operation your bones will continue to get weaker. Your testosterone levels will be lower than AFAB which can cause your spine to collapse or your hips to fracture. Getting treatment for that might require T when it becomes a problem.
So femboys don't have an easier choice but they just don't think about it and live in the here and now.

You need both hormones not just one. Your bones will lose density when you transition MtF no question. It varies per person what the damage is but there is definitely a risk and it definitely scales up as you get older.
The same goes for DVT and the myriad other complications unrelated to sexual function that nobody wants.
Lowering AA dose negates the positive effects too. They never really used to use an AA until studies were done that showed that using an AA had such a large impact on transition.
