The same way you get anything with exercise. Use either leg weights at home or machines at a gym, making sure you struggle slightly with the weight, and then lift repeatedly until you can't manage anymore. Repeat every other day, straining and building your muscle, until they grow large enough to be visibly different. You can get a visible increase in hip width with proper working out, at least an inch or two on each side.

I wish it were all sunshine and roses (those things exist, I even got some of both for my birthday), but that would be dishonest. Transition is hard, scary, and you want as much support as you can possibly get going in - friends, family, medical personnel, the works. Every step is terrifying, but rewarding. Coming out to the people closest to me left me feeling like I'd dropped a hundred pounds - no more bullshit, no more faking being someone for no one's benefit. Hormones have made me immensely more comfortable in my mind, and most days I look in the mirror, even if I still hate what I see, I hate it less than I did the day before. Kind of the opposite of pre-hormone mornings.
Still just starting social transition, but... when I'm just some plain, tall lady to the people I interact with, it feels comfortable in a way dealing with people has never, ever been.
I'm so much more self-conscious now, though. I worry about my appearance constantly, and getting clocked is devastating. Even just being "sir"ed or "man"ed in boy mode hurts my feelings. I get lonely much more easily, and I need physical intimacy on the regular or I start going a bit weird. So yeah. Support networks are very helpful for staving off madness. This place helps most of the time, too.
Having other trans friends to discuss trans stuff with keeps the isolation at bay.

Some (Suporn famously) won't do it at all if you've had an orchi, whereas others have preferences in terms of how the orchi incision is made so they have as much unscarred tissue to work with as possible.
