When I was a teenager I had the same problem. Erections often physically hurt and to this day I have no idea why. After HRT it went away but I started at 16 so it could have just been a growing pain or puberty thing.
They hurt because the skin on your dick is literally stretching.
You have it happen more often when you take hormones because your dick gradually shrinks, since the cells hormones are basically telling it to act like a vag.
