They usually mean someone with a different gender identity than what was assigned to them at birth, typically people with gender dysphoria who are driven to transition either socially, physically, or both as opposed to someone who assumes the label on a whim as a get-out-of-privilege-free card (think Eric Cartman in "The Cissy"), but essentially functions as the trans version of political lesbian while using "real" trans people as their sociopolitical shield for whatever personal or political agenda they have.
According to this criteria, however, Caitlyn Jenner is a "real" trans person.
Nevertheless, she's also an outspoken pro-life, anti-gay marriage conservative Republican which pisses off a vast majority of the feminists and LGBT people in the Democratic establishment. Nowadays, "realness," according to devout liberal queers, depends on whether you vote for Hillary Clinton or not, and no longer has anything to do with your self-identification, medical diagnoses, behavior, or treatment.
This confuses people of both major camps because they're trained to assume that trans people are a certain way. Conservatives think it's caused by liberal degeneracy, and liberals think it's some form of trendy, anti-conformist performance art. When trans people fail to conform to these physical, political or social expectations, they're seen by society at large as "fake," especially since many are already seeking to reveal them as "fake" to begin with, no matter what they believe or how they act. The media has, through a few short years of vapid inspiration porn and reactionary conspiracy theory, attributed to trans people a false perception of massive power and influence both from and over society. Everyone loves being the intuitive little boy who calls out the sitting Emperor for wearing no clothes, so that's why so many people are tripping over themselves to do so now.
