To get hormones from a pharmacy you need a prescription. If you want to go the self med route you can order online.
I'd strongly recommend that you have a plan for getting on a prescription at some point in the future if you're going to self med, having your levels monitored makes sure you're getting enough hormones.

the heavier your are, the more surface area comprises your capillaries and arteries and veins, therefore the more medication you need per KG to saturate your bloodstream to the point where said medication is effective

I'm looking into the research on it right now actually. Not that this is medical advice, but if it's a hormone, it's looking like the dose won't matter a whole hell of a lot. As a general rule with medications, particularly when working with either the elderly, the very young, or those that have unusual BMIs such as yourself, we pay attention to weight in regard to dosing. But I'm going to own up to being likely wrong on this one. The big caveat here is that there isn't a lot of research into optimally dosing hormones in HRT, because medicine places it at about the same priority as treating split ends.
