There's actually several aspects of male/female which don't always match up. There's gender roles/gendered traits, there's gender identity based on brain structure, there's sex hormone levels, there's anatomy, and then there's chromosomes. Most doctors would go by anatomy for defining "biological sex", while for most social interactions it would make sense to by gender expression.

First off, it's the case in enough of a minority of transgender persons that there are massive deviations in neurological development from the "normal" of one sex, more resembling the "normal" observed in the other sex in... every participant in almost every study published on the topic thus far.
Moreover, since tissues and bodily processes become increasingly those of the target gender as medical transition progresses, there's a lot less "isn't but looks like" and a lot more "is, whether or not it looks like".
So, we can say that the VAST majority of the transgender population are not "normal" (sex)s transitioning to "what looks like a (other sex)".
