Possibly start with 1mg daily doses of Estradiol, and 25mg bi-daily doses of Spironolactone.
Other than the psychiatric process, remember that your endocrinologist probably wants to test your blood for any medical concerns before you start, as well as knowing about illnesses and diseases in your family history.
As for getting the hormones, I can't help you there. I was prescribed both Estradiol and Spironolactone by my endocrinologist, and my insurance happens to cover the costs as well.
