I wonder if a folding or telescoping throw handle would work. Like a handle that folds out that you grip whole handed, or a rod that slides out with a plate or sphere on the end that you grip with the fingers. Finger grip might be too loose for something as weighty as a grenade.

Nope.
A 'normal' hand grenade relies upon your wanking-grip being around the body of the grenade to hold the fly-off lever in betwwen the moment you pull the pin and while you are getting ready to throw.
Then after it leaves your mitt on throwing, the lever flicks off, the striker hits the primer and the delay - usually 3.5secs - starts.
The pic related stick-grenade had a pull cord igniter in the base of the handle that you pulled just before throwing.

How are you going to hold the spoon in place when it's on a stick? You have to hold the spoon in place tightly. The moment it moves, you either better have thrown it already, or you have 3-5 second to get away and hit the ground. Otherwise, you're dead.

If it's a newly designed stick grenade (for whatever reason) then just put the striker and spoon on the grip.
If it's OP's ghetto rig then add an extension to the spoon so that it reaches the grip. I wouldn't like that very much though, since the extension could easily break since the whole thing is ghettod
