There wasn't even an EEEEEE afterwards for me, but that may have just been from adrenaline. No permannent damage, when I went to MEPs a month afterwards my hearing results were better than everyone else I talked to about it there.

Was at an indoor range shooting an AR-10 with just the earplugs. I didn't put them in 100% correctly and was in the first lane so the wall was right there directing all the sound into my left ear. I should have stopped after the first round and gotten ear protection but didn't think much of it, after 20 rounds everything was muffled. Luckily I have very little hearing loss if any more than before. Both ears seem to hear at the same level too so that asymmetry didn't happen. After it happened I bought ear plugs from the store because the muffling was so annoying and I figured the best way to heal the issue was letting my eardrums rest.
Tip: Roll the earplugs in the palm of your hand until they turn into a cone, then place in ear and let them expand to fill in gaps, do not cram ear plugs in.

I always use earpro when shooting. If I have to use a gun for defense, then I'm not going to worry too much on that occasion. That said, it's not a bad idea to have active earmuffs hanging around.
Also, a 20 dB reduction doesn't magically prevent all hearing damage. Having shooting as a hobby will do damage to your ears.
Put thousand of rounds of 556 down range on one day. With no earpro.
Not all shots are fired by me, but we all stood pretty close.
The temporary eeeeeeee won't kill your sense of direction or whatnot.
And I definitely do not suffer from permanent hearing loss.

Grew up hunting without hearing protection so I'm not too worried about it. I use it at the range to try and conserve what hearing I have but every now and then it's not a bad idea to fire a round or two without any hearing protection in. At least do it once before you have to fire your weapon in a real situation.
You need to save your hearing but you also need to be prepared for what it's going to be like when you fire that weapon without protection.
