From what I've seen from people I've met, the first one is definitely true and thus the second is even more true than usual because they suck to begin with and shooting is even harder under stress, and number three is because shooting someone typically doesn't kill people instantly unless some very specific places are hit (brain stem, major artery/vein, spinal cord, etc.)
