The killing is a classic example of people ignoring gut feelings because we're conditioned from childhood not to be offensive to people. He didn't even need to cancel. If they'd just watched the dude under the guise of being safety nazis it would have been avoided.

He racked up an admittedly impressive service record, and then bragged about it because he knew people would idolize him as a hero in an otherwise contentious, oft-debated war. He was unquestionably dedicated, just as he was arguably neurotic. I don't think anyone taking an objective look at the man can deny some narcissistic behavior, at the very least.
For as much as some like to portray Chris Kyle as a down to earth and all-American badass who drank from the keg when he wasn't bleeding on the flag to ensure the stripes stayed red, the vibe I get from watching his interviews and reading his book (ghost-writing debates aside) is one that would have probably made him a chore to be around for any sustained period of time. He just didn't seem very likeable.
Placing a belligerent Type A personality in the same truck with a mentally unstable, shell-shocked war vet with an inferiority complex wasn't going to end well when you filled the bed with a Texan-sized gun collection and drove out to a secluded range.
No medical professional in their right-mind, no matter how pro-fun, would have thought that was a good idea without at least stopping to ask if all parties involved could be trusted around that kind of hardware. And if you're war-wound wasn't physical, I'm going to hazard a guess and say no.

Thinking about this some more I see three events where the tragedy could have been avoided.
First is Kyle not shooting with guys with issues at all. Like you mention not a many people with backgrounds in treating mental health issue would recommend it but I imagine Kyle would say that's just because they're liberal pussies. Fact is though a lot of those guys shouldn't be around weapons as they aren't in their right minds. It's admirable that he tried to connect with them over a shared hobby. That said some selectivity would have helped. Which brings me to number 2.
He shouldn't have ever taken that guy shooting. Assuming that it's perfectly fin to take vets with PTSD/difficulty adjusting back to civilian life shooting he should have used some kind of vetting process, at the very least consisting of meeting with the guys without shooting first. Everything I've seen about the guy that killed him suggests that he had mental problems from before he joined. His family probably tried to use that as an excuse. It less stigmatizing to have PTSD from being in the service than to have been nuts before. A simple lunch or evening chat would have let Kyle pick up on the "this dude is crazy" vibe before he was on his way to the range.
Finally the point I made earlier about range safety. Kyle shot a lot. That tends to make you pretty casual about safety procedures. Not a smart thing to do when dealing with some who is potentially unhinged. If you're going to engage in dangerous activities with people referred to you because their family is concerned about self destructive behavior you should be extra careful. Act like you're dealing with young kids. One of the two should have been constantly watching him just in case. They knew this, hence the "watch my six" text. Kyle and Littlefield shouldn't have been out there with the guy in the first place but if they'd trusted their gut and been a little more cautious they may could have prevented what happened.
