The marine flare inserts are not designed for shotshell pressures. Inserts for normal cartridges do exist, but not in 12 ga due to that being over 0.50in and being a destructive device if rifled (they are not exempted currently) and being a NFA item if smoothbore. The flare inserts are lettered by the ATF on the premise of being a signalling device, same as the flare pistols sold in stores.
The largest lettered insert for cartridges is 45LC/410 and is rifled. They are from Runway, others may still make them. Sight adapters are available, but accuracy is poor regardless. Also, the rifled inserts cause the shot pattern to spread very quickly if using a shotshell.
Also, possessing the flare gun and the insert is constructive possession and constitutes a firearm, whether or not it is together. Assembled, they are an Any Other Weapon.
There are certain situations where these make sense, usually when in a National Park or boating, where a flare gun is normal but firearms are restricted. Beyond that, they sort of suck.

I have a 12ga to .22lr rifled adapter from Numrich if anyone wants to buy it for $15. Took a grinder to the end so it would fit in my Olin flare gun. Worked fine until I accidentally busted the flare gun itself while trying to get the adapter out to clean it.

The 12g are made with 12g flares in mind, not shells. You could get an insert in 410 if you want a shotgun pistol though.
That is a good price on that flare gun though, I'd grab one before they are sold out if I didn't already own one.
