What does modern self defense pistol doctrine say about lanyards? I had to use one when I was standing quarterdeck watch when I was in the Navy. Should you use a lanyard when OC or CC? Or do modern operators think they aren't high speed?

They are okay but useless in any situation where they would be considered a useful item.
Most useful when shooting from horseback is likely. Also possibly useful when shooting from a vehicle. Otherwise I'd think they'd mainly be annoying.
I wear an old ALICE belt for hunting and tie my 1911 to it so I don't drop it. It's better to just get a good holster (all of my money is going towards more important things desu) but the retention strap as is called the modern day lanyard is a good "safety net" so to speak.

Lanyard are usually to prevent losing the gun (theft, being picked by mobs, fall into manhole or something, etc). LE in other countries (Hong Kong, Japan, Malaysia and few others) still use them.

The main use was to keep straps for packs or other stuff (like ammo carriers and whatnot) from slipping off the shoulder. Also, of course, to attach your elaborate gold epaulettes.

I don't own any real pistols, but my airsoft pistol once fell out of the holster during a game, so now I always use the lanyard.

I use a pistol lanyard to keep those fancy leatherman trauma sheers attached to me. They were a gift so id feel bad losing a pair of $70 scissors. Pluss i can pull them out and then drop them and not ever lose them
Unless you're on a boat or in water...
They're in case you drop your gun while running for cover or moving position.
That's really useful on a battlefield but doesn't seem that necessary for personal defense.
