You're correct. There are companies that sell shotgun adapters for flare guns, once you pay the tax. But, once you put the insert into the flare gun, ATF considers that to be "manufacturing" a firearm. Even though you can remove the insert,I don't know what that does to the status of the flare gun.

"Chamber" isn't really a thing with the flare pistols. The 12 gauge flare insert has a constriction about 2" in, though.
As for the price of the flares, the market decides. I was buying the starburst flares for $3.00 each at my lgs for awhile, and parachute flares for $10.00. When he ran out, he never got more.

That's not an extension. 12 gauge flare inserts neck down, so out of necessity the flares do as well. That's so people won't load shotshells into a plastic pistol and lose a hand. With the metal flare guns/inserts, you can't chamber a regular shell either, or you'll turn that pistol into a pipe bomb.
