Yeah he does, they shoot each other up pretty bad and kinda come to some agreement to kill each other, but the cop expires before he can pull the trigger. Then Rambo's CO shows up and blows his brains out. It got changed because by the time they wanted to make it a movie, PTSD was considered a thing and people were starting to feel bad about the way they treated Vietnam vets.
