He's reloading when not all of the rounds have been fired.

There is still a round in the chamber when he charges the weapon, so it ejects the round.

He is doing a tactical reload, so he doesn't actually need to charge the gun. I suppose it's easier for the animators as well as the manual of arms for Snake to always charge the gun when reloading.

I do notice that he's doing tactical reloads, so the previous mag wasn't emptied. Maybe the game engine is able to recognize that a round is still chambered, but they forgot to omit the charging handle animation bit when you reload from a mag that still had rounds in it?

No, when you first load a mag, you charge the gun so it chambers the first round, after that you don't need to charge it again(except if its a bolt action) unless your intentionally trying to empty the chamber

When you cycle the action, it performs the same action as if you were to fire the weapon, except the round does not get fired.

It will eject the round, whether it is spent or not. You can do this to clear the weapon, whether to make it unloaded, or to clear a malfunction.

Reloading while you still have ammunition in the magazine. You reload in order to have more rounds in case you need them for an upcoming encounter or threat. For example, you shoot some insurgents, and in the process fire like 20 rounds. You only have 10 rounds left. If there's a pause in the fire, you eject the magazine and load another fully loaded magazine in order to have 30 rounds.

Basically all of the manipulations you would do to the weapon, in order to use it effectively. People train to use a weapon fastly.

For example, ejecting the magazine, loading another magazine, and racking the slide on a handgun would be a manual of arms. You train to do it, and it's a weapons manipulation.

I think the wording is off. He is referring to a middle eastern technique of loading the first round by hand so you'd always have a round in the chamber. Note in this case, "You were trying to manually eject the first bullet by hand" should have been "you were trying to manually load the first round by hand."

The problem comes when Ocelot was apparently trying to do this after he slams the magazine in the magwell. What happens is this sequence:

No, he tried to rack the slide and eject the first round for whatever reason. The problem with the makarov is that you have to rack it a certain way or else it will jam. i.e. if you don't let go of the slide completely and let it return on its own.

I wonder why Kojima would do this, all games before used real weapons. There was a theory that maybe Diamond Dogs' R&D made them, but the Soviet soldiers also use frankenguns like AK with that aftermarket rear sight that was also in Call of Duty: Black Ops(pic related). Would check IMFDB for the exact name, but it was infected with a virus for a past few days, did they fix it?

Also there is a second theory that maybe Ground Zeroes and Phantom Pain are a dream or something.

Or maybe they just wanted to save money and didn't want to buy rights for gun names. Still, Ocelot using some no-name frankenrevolver looks pretty weird, SAA was his gun in all MGS games, those that took place before GZ/TPP and after, so it makes no sense for him to use another gun.
