I've been wondering how well-known Sound Horizon actually is there, I assume it's relatively big since they are able to make such elaborate live shows, but there's very little fan-art and such compared to other Japanese media. Maybe it just doesn't share the same demographic?

Sound Horizon is very underrated even for japanese standard. I think it is because of the anti-mainstream tendency and it is hard for new listeners to enter the fandom since Revo album is pretty much sophisticated for songs album with overwhelming chunibyoo
so people don't spread it like mainstream songs in Japan not to mention that sound horizon project is declined since revo focused on Linked Horizon right now.
The demographic today is about 20~40 years old people with 35% is male and 65% is female compared to the season 1 of sound horizon where most of fans are male.
But it has many appreciations from professional singers and the fandom itself will talk back about it when revo doing major release like story concert. it was proven when nein get 1st oricon chart for like 2 weeks
Tldr : It is not popular because people are not talking about it in real life

Nein is story about rejecting/denying past and creating alternate reality of happy endings from previous sound horizon song that is mostly ended up in tragedies
The story follows a view from an entity known as R.E.V.O. and his journey visiting archives of past horizons and his attempts of changing the tragedies befalling the characters present in those past horizons. The recurring theme in Nein is about interpretation, as well as one's own view in various subjects and aspects of life, a theme heavily based on the theory of Schrödinger's Cat.
In this third chapter R.E.V.O denied the song of "Rinne no Sunadokei"
Story is about woman who refuse to eat after losing her son and died while embracing her husband.
