What are the best plot-driven touhou doujinshi, whether games or manga?

I feel the same. The world is circlejerking about the massive fanbase Touhou has and all its contents it creates, but the official works blow the doujinshi out of the water most of the time. Especially plot- and worldbuilding-wise.

Pretty much this, there's a lot of shit that uses only their designs to fill whatever stupid and out of character role they feel like. You'd be surprised how often the scarlet sisters get the "LOL I'M EVILZ AND KILL YOU FOR NO RAISIN" role. Most doujin creators are secondaries on par with fanfiction.net or deviantart, the only difference is they have the talent for art.

My favorite official Touhou story so far is Cage in Lunatic Runagate.
Favorite doujin wise, it's probably Live a Touhou or Dream or Reality.

Anything that's "plot-driven" is by definition going to be somewhat secondary seeing as how somebody has to invent an original plot for it.
If you're into bomb-dropping plot where shit happens in or to Gensokyo, you could try Dream and Reality or Utsuho of the Void. (Tohonifun's stuff is worth checking out if you like dumb over-the-top shit, Koumakyou being my personal favorite.) If all you're looking for by plot-driven is anything with a plot, well, that's most works. Omoito series is a cute and light-hearted series by Morino Hon that's worth checking out.
Finally, if backstory explication or interpretation counts as "plot-driven" I suggest Though the Wind Cries (Moriya story), At Chirei (underground story, by the aforementioned Karaagetarou), and The Mysterious Tale of Asuka's Illusionary Bridge (a Ten Desires story).
Doujins about minor characters tend to have better stories. This Akyuu hentai doujin, for example, ruined my fapping session because it made me feel bad.

I actually don't remember very many of these and I lap up a lot of terrible EoSD doujinshi. Remilia is usually playing at evil and Flandre rarely escapes the house.
