get a sturdy wooden board and figure out a way to get it on an incline, resting it either on the wall or some other objects at the farther edge of the table. you can also have the flat monitor at the farther higher edge of this incline by different ways. how you do that is for you to figure out depending on factors that are particular to your situation.

no shit it's gonna hurt because you have to lean forward to look at the paper closely. The reason why people draw on boards like in the OP is because it avoids this problem.
Ideally you should draw like the pic in the OP though to avoid back problems.
