Take one or two commissions under the table, and let whoever know that you have a backlog and it may take a little bit of time to get back to them. Get what you need to pay your shit but CLOSE YOUR COMMISSIONS after the fact. If this is your only source of income, just hustle. It'll mean 8-10 hours or more of work per day. Just do it.
Obviously, have a hard cap on commissions later. No more than 3. When you do catch up, be more transparent. Let people know where you're at with the work and aim to have a turnaround time of only a week or so. This will make you more money because you won't be clogging the pipeline and you'll develop a reputation of working quickly. Faster output means more portfolio material as well.

I don't know anything about your "hard ass time" but you sound like you're just making excuses to be brutally honest family. Saying how you feel bad, or just can't do something or forcing things and how it's all so hard and you feel guilty but oh these things out of your control (hard time, muh ADD, etc) all sounds like you're making these excuses to yourself even more than to us or the clients.
I've known artists of decent skill level who've crashed and burned because they lie to themselves about how they will catch up on their commission list, really they will, but they have to sort things out first, or find the time, etc etc. These same people sometimes left commissions undone for almost 2 years.
There will never be a good time, life will never not be hard, your "ADD" will never not kick in, you'll never not have to force it and try hard to get shit done. That's the difference between a professional and an amateur. It's not about how well you paint or your experience or paycheck, it's about knuckling down and getting the work for the client done on time, because you're a business and if you don't provide the right services and on time then you're worthless.
Separate yourself from your work more, stop making excuses and lying to yourself, and either knuckle down and get the commissions done starting TODAY, or outsource them and don't take on any commissions until you get your act together.
