Are there a lot of historical examples of fighting to the death as a sport? Any places where this still takes place today?

While the Romans were the meme with their elaborate Games, they weren't the last people.

Early half of the medieval age, Tournaments were quite literally battles in which people died or could die. You could die on a lance or get brained ina melee. In China, the Lei Tai (fighting platforms) hosted mostly martial arts combat and dummy weapons, but the highlight of any Lei Tai was a grudge duel between two olffended parties with real weapons. Since Chinese law demanded duels to take place in public locations so as not to make it a murder.
