The only confrontation, in a career that spanned both 20th century world wars, he categorically "lost" was a guerilla incursion to release PoWs early in the Battle of the Bulge, and that was a few hundred ( ? ) men. (He wasn't present, but it was under his order)

I think a lot of it stems from rivalries with other generals. I remember Patton being considered a very aggressive commander that conflicted with his fellow generals such as Montgomery.
