First, that particular gag is often loose, although it always *feels* tight when I first put it on. Maybe I'm just a situational moron when it comes to that gag, because I've never had this issue with the other gags we use (cleave, ball, and bit, in descending order of frequency of use). Now that I think of it, though, when I've put that gag on her, it has (for whatever reason) always been when she's flat on her stomach. Maybe it's her weird neck/head angle in that position that distorts the actual circumference. Hmmm....
