There is a list of keywords which are monitored and logged if they appear somewhere on the internet. If the NSA wants to know more about the source of the keyword, they go to the provider and ask for serverlogs (and usually have the right to get them) or they use analytic programs or honeypots to colloect informations.
Rule over the thumb: If a web service is based in the US, it's easy for any government argenture to infiltrate the service or get server logs. Reading any chatlog, shitposts, tweet and so on from every american would bea job even the NSA or any other government has the workforce.
For normal people they will very likely just look at what sites they browse, what numbers they call, what people they chat with and if there is infact a overlap with someone that is/was on a list expect to be on the list very fast too.
Anyways if you are rich, influential or in the politics you are very likely watched by the NSA.
