The company I work at outsourced once. The code was horrible, sluggish, impossible to maintain etc. It spawned a different thread for almost every chunk written by someone else and then just joined it. Sometimes even had 40+ threads running for no fucking reason.

Certainly Russia. Not that that stackoverflow proves anything. It's just that they don't have a culture of believing bullshit for the sake of it.
They certainly don't get noticed enough. The US get noticed the most because they have their fancy funding for their fancy software projects. And as with any big place there's plenty of good programmers. But from my experience with Russian programmers they got the highest density of quality programmers that I know of.

Really? My experience with Russians is just uncommented spaghetti code that has nonsense names for functions/classes. For example, a1__i was the name of a class that literally only existed for one public member function.
