It's Bottom Slider, a lock screen designed for use with Smart Launcher. It's pretty annoying actually, in that I find it to be one of the best looking lock screens out there, but if you don't use it with Smart Launcher then the music controls on it stop working. I have been looking for a solution to this, either by fixing Bottom Slider or by finding another lock screen that I like equally as much.

Clock is made in KWGT, then modded to work in KLWP. Google now launcher doesn't support icon packs by default, but with exposed you can install XGELS and change them.

For the icons, I opened my background in Paint.net, filled n the gears to remove what was originally there, and put Zwart icons in the gears. Then in KLWP, I put invisible circles over the icons and set them to open the corresponding apps on touch
