Three things, Nvidia didn't do research on TSMC 40 properly, Nvidia made an architecture with too little encapsulation, and the voltage chosen was completely inappropriate.

Rushed to market. The 500 series refreshes of Fermi were much better cards, with more sane power draw. The even managed to get the fully-unlocked 512 shader GF110 to market, after having to hold it back during the 400 series because heat and power consumption were so far out of control.
So yeah, after some further refinement and development it was a fine enough architecture. Just Nvidia trying to meet deadlines and beat AMD to market killed it.

It took 14 months after the HD5870 series had already been out.
And the 5870 still competed with the 560 Ti easily while using less power.
