Minimized windows are in the right of the dock, but you could just hide the application like a sane person, so when you unhide it, all windows are unhidden.

If you were doing that it'd show multiple icons. Most applications will never do this and the only one I can think of off hand is mpv launched through the terminal.

Minimized windows appear in the dock. (Pic related), and you should be using mission control to see what's in the foreground and background

Actually I figured it out.
checked = in focus
diamond = minimized
nothing = not minimized && not in focus
