They don't know what extensions you have installed, only plugins.
Checking for adblocking is simple. You just have a 1st party script that checks for the 3rd party content, or you have the warning as default content which gets removed by the advert script.

Video players are quite a bit tougher because they can incorporate the content display and adverts all in one.
The player just goes "I didn't get a video back when I requested an advert so I won't play the content video".
Perhaps if you can provide an empty video of suitable codecs/etc to redirect to instead of the advert video you could get near uninterrupted content playback, but that's beyond simple blocking.
Ads are known to distribute malware. Block them at all costs.
