I live in Portland Oregon and was a little freaked out when I went into one. I didn't realize I walked into one and used the urinal. Suddenly a woman walks in and I get spooked thinking I went into the wrong bathroom for just a moment then realize wait I'm using a urinal though. I walked out of the bathroom and investigated the sign and realized it was gender-neutral. Besides the moment of shock it didn't bother me to much honestly.
It would be a more efficient use of bathroom space.
