I've only ever heard of this happening when someone clicks on the "submit order" button too much, and they don't have much money in their account, it goes into overdraft, and since most poor people don't opt in for OD protection, they get raped with fees.
Paypal used to solve those problems and give people the money back even though it wasn't their fault, but apparently they stopped and pushed it onto banks - which of course banks are retarded so they don't help.

I didn't notice, made a $30 purchase, then paypal continually tried to draw payment from my empty bank account because my debit card was empty. Two weeks later my debit card had the missing $45 back and my bank account had a negative balance of $250
