Every board has janitors. They receive reports from users who use the report function, or that they notice while browsing the board. They can delete threads/posts and mark them for a ban. If they mark something for a ban, a moderator then reviews the moderation and proceeds to either ban the IP in question or not.

I think last time I was banned year ago. Back then on banned site I could see reason and link to the thread or post that was reason for ban, but now I only see Gloobal violation and no evidence for rules violation
