it works better with the keyboard in your lap and the thing holding your mouse should be low enough so that your shoulders don't bunch up and it should also have a pad on it for your forearm to rest on so that your wrist stays slightly elevated, this keeps your wrist from bracing the weight of your arm. you also want your feet elevated, an armless recliner would probably be your best bet.

When you rest your arm your shoulder should be neutral neither bunching up nor hinging down it's quite simple really. Just looking at that setup I can see my shoulder bunching up but maybe your arms are a different length or something I dunno.

your arms shouldn't be angling out either. everything you do to compensate means some other part of your body is bracing additional load but having your arms angled out may be one of the lesser problems you can have, especially if it works for you but you seemed to indicate that it did not work out, why?

I think it's great as far as using the mouse is concerned because having your arms more angled means you can have your hand in a more natural rotation while holding the mouse

yeah I had a setup similar to yours and one of the features I made sure of was that the uh.."bench" the mouse rested on was about level with my lap, where the keyboard sat so that I could seamlessly move my hand from mouse to keyboard. I'm not sure what you mean about your hand being in a natural rotation. you ideally want your hand gently draping over your mouse so as to not put any pressure on your wrists or palms and to maintain ideal control of the mouse.

Oh god vertical mice are terrible. I'd highly suggest a trackball before trying one of them. Your carpal tunnel was almost certainly caused by incorrectly using your palm or wrist to brace the weight of your arm against your table or mouse. A vertical mouse may solve that issue but now you have your ulna and elbow facing down and no longer have the meaty part of your forearm to support with.
Really a trackball or hell a motion controller but I'd fix my table height to arm rest height ratio ergonomics first. A proper setup will place zero pressure on your carpal bones.
