You can use this to calculate your power needs. Might have to choose another similar CPU from that list though as yours is quite old. Anyways I doubt your problems are originating from a lack of power, unless your PSU is broken; 750W ought to be more than enough for your set up

The system is to old for upgrading. You would just be throwing money down the drain trying to find parts that fit in there. It would probably be better to start a completely new build instead

you can keep the monitor and optical drive - maybe the HDD if you're really desperate. and possibly your case and PSU.
other than that, there's no way to upgrade that anymore, build something new.

By upgrade you mean replace I assume. If that board was am3+ i would recommend a cheap fx-6300 but it is not and probably doesn't have the proper VRMs to handle FX. That said,
Cheapest lga 1150 core i3 cpu -$100-$120
Cheapest h81/h87/h97 chipset motherboard -$50-$60
8gb ddr3 1600 -$50
Buy a video card used. Which one you buy will come down to how good your power supply is.
To be honest though, if you just save more youd be better off. $750 is more than adequate to get a decent built that will play latest titles and last a fee years.
