What browser are you using? Extensions and "apps" in Chrome/Chromium are known to let ads through and there's nothing that other extensions can do about it.
For example, neither Adblock Plus or uBlock Origin will be able to block all YouTube ads if you have the YouTube app installed.

Update your fucking hostfile instead of installing some bloatware that's just gonna get paid off by another ad company...
What sites? The only ads I see get through are ones hosted as normal images by the site I'm visiting (eg referral links).

Ads in video streams are probably the easiest to get around adblocking software.
