I would assume a well compiled Gentoo would be the fasts Linux your machine could achieve. However, I doubt it'll actually result in much perceived speed improvement.
As for GUI, if you want ultra slim and fast you'll probably want to avoid a full fledged Desktop Environment altogether.

Linux is a kernel. There is no GUI. I'd say that purely a kernel, with no other software, is pretty fast/slim.
install slackware 2.0 and install only A, N and X disk sets. It should run fine on any 386/486 pc and it will be the smallest working linux distro with a GUI.

Lubuntu ot Puppy. Though I'll note I don't have much experience outside of the Ubuntu based distros.
Just installed Lubuntu on my Thinkpad T30, and to me actually feels usable without having to be patient because it's an old machine trying to keep up. It'll actually play HD 720 videos, whereas others might struggle to even play 480, Some options I usually expect to be in a control panel setting I had to do via the command line, but otherwise like it so far. If this Thinkpad didn't have a 802.11b card, I'd actually consider carrying it around.
