It depends on how ugly you are. For most people something as simple as losing weight and taking better care of their skin is enough, for others, not so much.
Plastic surgery is of course a huge life changing decision that shouldn't be taken too lightly. There are several procedures that can be used by a plastic surgeon to give you the look that you want (you can find some by using google).
I would say, try everything possible in your power to change your looks. If it doesn't help much, then surgery is a possibility.
