Okay /fit/, I'm in a bit of a desperate situation and I'm asking for help. My mother underwent quadruple bypass surgery Wednesday morning and she's been unconscious ever since. Her blood pressure is at 35 over 19 and the medication she's hooked up is the only thing keeping her alive now. The problem is the medicine keeping her blood pressure up is restricting her kidney so she's close to complete renal failure. Any ideas what type of medicine we can use!? They used lasex and it failed.

It's not looking good. I've been sitting beside her on and off for the past 48 hours. It's a miracle she's still alive. I tried asking Reddit and I got jack. Some of the answers here got me thinking of different questions and routes I could pester the docs with but no avail. I'm tired and now I'm waiting, it could be few hours or maybe a day. Thought you'd like an update. I've taken the docs aside and asked the tough questions and I'm prepared for the worst. Organ failure and brain damage are all she looks forward to if she survives. The real suffering is my dad who left my mother alot of financial responsibilities. I'm going to be very busy. Thanks.

My dad passed away on (of all days) Fathers Day this past June. He was in the hospital for 2 weeks yet no one on his part of the family told me anything (i have a bunch of half siblings). I received the news on Fathers Day of his condition. I prayed to God to let him die in peace and not to worry about me and his family. That we would be ok and if it was possible to let him go to Heaven if it meant Id have to go to Hell. He died like an hour and a half later.

Be thankful and spend time with her before she goes.

They're giving her lasix to decrease volume and reduce strain on her heart. In the acute state CHF and shock, lasix actually increases blood flow to the kidneys.

My mom had an aneurysm and I found her spasming on the living room sofa. She had a DNR, but we moved several times since she wrote it and we couldn't find it, even after turning everything upside down. She usually took care of all the paper work, but it wasn't among all the other stuff. After waking from the coma, which took a few weeks, she was basically a vegetable, but she was awake. She would make weird gestures with her hands and couldn't speak nor recognize us. She would moan a lot, though. I know for a fact ending up like this was her worst nightmare. Paying for the care of a husk almost ruined our family financially, and when she finally died by an MRSA, all of us were glad.

There are worse fates than death. I wish I could have been sad, when my mom died. Sorry for your loss, OP.

Lasix is taking fluid from her pump and dumping it into a Foley bag and any fluid they add to her just goes right there anyway. Any pressors she's on are just squeezing a pipe that's fucking empty anyway. Pressors themselves have a negative impact on kidneys due to renal shunting but you can replace kidneys before replacing the patient. I'd personally forego her kidneys and fuck them up and have her on dialysis than let her sit without enough Systolic Pressure to perfuse her brain. Cardiogenic shock seems unlikely unless there's pericardial rupture from the surgery which happens but I'd imagine they would already have cracked the chest by now if that was the case. I'd go for sepsis and hypovolemia first of all given the information presented but you just live in your resident program mindset and think that the world works exactly how you think it does if you want.

Sorry OP but your mom is probably on too much levo to be weaned off any time soon and probably isn't gonna make it. It's sad but it happens and I've seen it happen many times over the years.

Alright guys things have wound down and family is all relaxing and eating. Turns out you guys were the most useful among the communities I turned to and helped me gather my thoughts and ask the right questions. I appreciate the frankness and it steeled me to the inevitable.

About my mother; she was 55, a smoker, and out of shape and gaining weight. To make this /fit/ related my doctor told me I was prediabetic so I've changed my diet and exercise habits thanks in part to you guys. I lost 50 lbs, gained a little muscle, started feeling better, and reversed the onset of diabetes. Part of what my mother was going through helped motivate me to get in shape. I tried many of times to get her work out with me but she'd never tried. She had 99% blockage so in retrospect she never had the energy. It was a cascade of events that lead to the complications over the past two days stemming from her poor health. So stay /fit/ for yourself and your family. Thanks to you guys I'm the healthiest in the family and the extra energy from being in better health helped carry my family through all this.

BTW I've expected the banter, doesn't bother me at all.

Electrolytes don't help blood pressure, hypertonic solutions simple pull fluid from the vascular space to fill the vessels. If there isn't much there they won't do jack shit. Lasix is already causing the shift and will mess up Electrolytes but the woman probably already has a peg tube in at this point to renourish the body of lost nutrients. Besides she's probably accompanied by an amiodarone drip because I'm sure she has post operative a fib due to the Lasix fucking with her potassium

My dad's got bowel cancer and the tumors in his abdomen are tearing him apart, morphine syrup is the only thing keeping the pain away.

You too. There's absolutely nothing to support sepsis or hypovolemia at all. If it was due to blood loss during surgery they would have pimped her full of fluids and RBCs already. If they're using lasix and pressors it's due to pump failure which is the source of her low blood pressure, not hypovolemia. They're trying to decrease her volume status to improve her blood pressure. Go back to reading webmd and pretending as if you have any idea what youre talking about.

It really depends on what kind of SHOCK she is having. I would guess CARDIOGENIC because she just had heart surgery. She's probably on dobutamine and epineprine? Or... is she having SEPTIC because she has an infection? In which case she would be on norepinephrine and vasopressin?

I really can't tell what kind of situation your mom is in, but a blood pressure of 35/19 is not compatible with life. She'll probably need dialysis if her kidneys continue to fail. It could be temporary if she has a good prognosis.

It depends on her co-morbidities, too. Is she obese? The fatter the higher risk of death. Uncontrolled diabetes? Considering she needed multiple vessels bypass'd, she probably didn't live a healthy life.

I'm sorry that your mom is in this situation. The medical staff is probably doing all that they can to keep her alive. The brain is the most important thing to keep alive, even if that means that the kidneys need to suffer temporarily (hopefully).

If she's not getting enough oxygen to her brain for too long, she's going to have brain injury and will never be the person she was before. Even now, you can't expect her to go back to how she was before.
